Whether you’ve been with us for years or just recently joined, we’re honored to care for you and look forward to celebrating you!
WHO IS ELIGIBLE?
All current patients of record, from birth to age 18, who have been seen by one of our providers within the month are eligible.
HOW DO I ENTER?
- When visiting our office during the months of May, June, or July – your child(ren) will be eligible to enter.
- Each child is eligible for one entry ticket per visit.
- Entry tickets will be provided to parents by our front staff. The tickets will require: Name of Child; Parent Email; Parent Phone Number
- A winner will be selected monthly at each of our office locations.
Not visiting during the summer? No worries! We will also be holding giveaways for the other seasons too – so there will be more chances to win!
ARE THERE ANY FEES?
No, there are no fees to be entered into the drawings. It’s a gift from us to you!
DO I HAVE TO PARTICIPATE?
No, you can decline entry into the giveaway.
WHAT ARE THE PRIZES?
Each month, we will draw for a custom Dentistry for Children tote filled with goodies!
CAN I WIN MORE THAN ONCE?
If you have more than one visit in the month, that means more entries and the greater your chances! All names entered each month will be removed for the following month’s drawing.
IF I WIN, HOW DO I GET THE PRIZE?
Prizes will be available for pick up only at each office location.

Who is eligible?
Children ages birth-18 will be eligible to enter.
How to enter:
Fill out entry ticket at their dental visit.
One entry ticket will be given per child for parent to enter information.
Entry tickets will only be valid for one draw cycle, after which entries will be discarded appropriately.
Conditions of entry:
Must be a patient of record and up to date on routine dental cleanings (seen within the last 6 months).
There is no entry fee or purchase necessary to enter the drawing.
Start and Closing Dates:
We will start entry at the first of the month and close the last of the month.
Details and values of prizes:
Monthly prizes will vary each cycle.
No cash alternative to the prizes will be offered.
Prizes are not transferable. Prizes must be picked up from office.
Notification of winner:
Winners will be notified via the email / phone number they wrote down on their entry ticket.
Winners will be selected at random from all entries received.
Winners names are final and no correspondence will be entered into.
Winners will be notified within a week of the prize drawing and advised when and where the prize can be collected.
By entering the competition, the entrant is indicating his/her agreement to be bound by these terms and conditions and consent received by parent/guardian.
Winner will be announced through our Social Media channels and only first names revealed (Facebook and Instagram).
