New Patient Forms
Although our waiting room is full of fun and entertainment, we’re sure you want to get in and out of our office as efficiently as possible. To make your check-in process as smooth as possible, please take some time and fill out our online forms before you arrive for your appointment. That way, you’ll be ready to get started with your visit in no time. There are two ways to complete the forms prior to your scheduled appointment. The first is through your XL Dent account by viewing the “Patients” tab and selecting “My Account”. After logging in, your needed forms can be accessed there.
